By creating your own Personal Medical Registry in advance, you protect yourself and your family from delays that can be life-threatening. At Monroe Hospital, we keep this information as an active personal record just in case of an emergency.

When you complete your Personal Medical Registry, we have a more thorough description of your medical background, including blood type, physician contacts, family emergency contacts, precise care requests, insurance information, organ donor cards, immunization records, etc.

What does it cost?

Absolutely nothing. At Monroe Hospital we maintain your Personal Medical Registry at no cost for you. This includes your immediate family and extended family. It is impossible to place a value on information that can save lives, so we store this information as a free service for you.

You will be sent a FREE ID card …

Your FREE identification card tells how to locate and view your Personal Medical Registry file. This allows a medical caregiver to access your file and begin care. Armed with this additional knowledge, they will be in a much better position to provide timely and appropriate care for you - possibly life-saving medical care!

Completely Confidential …

Your Personal Medical Registry is maintained at Monroe Hospital with the same confidentiality as a medical record.  All information entered on this site is protected using a Secure Web Site Certificate to ensure privacy and protection.

How to Register?

1. You may register online and complete your Personal Medical Registry NOW! Click here to complete your Personal Medical Registry.
   
   
2. You may call 812-825-1111 and request a form to be mailed to you.  You may complete this form in the privacy of your home, then return it to Monroe Hospital in the pre-paid envelope.
   
   
3. You may send an e-mail to medicalregistry@monroehospital.com to request that a Personal Medical Registry form be sent to you quickly.  Again, you may complete this form in the privacy of your home and return it to Monroe Hospital.
